Birthrights has published a new report on coercion

Birthrights has published a new report on coercion in UK maternity care.

They heard from nearly 300 people including women, birthing people, midwives, birth workers, and community groups. In 2025 alone, 112 of the service users who contacted Birthrights' advice service, which is more than 20%, reported coercive practices.

The findings are stark. Structural racism and discrimination embedded in maternity care. Blanket application of guidelines overriding personalised, informed decision-making. Inappropriate safeguarding referrals and threats of social services involvement used against women who make choices outside hospital guidance, disproportionately affecting Black, Brown, migrant, and marginalised women. And a blame culture that leaves midwives too afraid to support those choices.

The report also names what many midwives already know: that coercion doesn't only harm women. It’s driving midwives out of the profession.

ARM has worked alongside @birthrightsorg on the Cahill joint open letter and has supported their campaign on the Gloucestershire homebirth suspension. Birthrights will be with us at our 50th Anniversary Conference on 20 June at UCL.

Continuous care from community-based midwives redu

Continuous care from community-based midwives reduces the risk of preterm birth by 45%.

New research from King's College London, published in BJOG, analysed 6,600 pregnancies in South London. Women who received care from the same small team of midwives throughout pregnancy, birth, and the postnatal period had significantly better outcomes than those in standard care.

The benefits were greatest for women from Black, Asian, and ethnic minority backgrounds, and those in the most deprived areas. Preterm birth risk dropped from 9.5% to 6.4% for ethnic minority women and from 8.2% to 5.1% for those in deprived areas.

Despite this evidence, NHS England's plan to deliver Midwife Continuity of Carer nationally has been on hold since 2022. Women cannot currently choose this pathway, and the model is only available in some Trusts.

Optimal Cord Clamping.
Whose blood is it anyway?

When a baby's cord is clamped immediately after birth, approximately 30% of their blood volume is left behind in the placenta. That blood contains iron, oxygen, over a million stem cells and goodness knows what else them at has yet to be uncovered. There has never been any evidence to show immediate clamping is safe for babies, only evidence that it causes harm.

Registered Midwife and general Nurse @amanda.burleigh has been campaigning for optimal cord clamping since identifying that immediate cord clamping g had no evidence base in 2005. She co-developed the LifeStart trolley, the first bedside resuscitation device that allows vulnerable babies to be resuscitated with the cord still intact as the evidence shows that premature and compromised babies benefit from intact cord resuscitation. She's watched international guidelines change from globally recommending immediate cord clamping to delays of approx 60 seconds (which is not enough) because of this work. She's also the founder of the “waitforwhite” campaign, co-author of the #SayNoToBullyingInMidwifery report and is working with the BabySaver team, which is a small tray which aids facilitation of intact cord resuscitation.

In this ARM webinar, Amanda presents the evidence and discusses the changes in practice, making the case for optimal cord clamping and intact cord resuscitation and how we still have a lot of work to do to ensure all babies receive their cord blood and how midwives can contribute to ensuring babies are born as healthily as possible.

A national survey of 3,682 women has compared mate

A national survey of 3,682 women has compared maternity care models and their outcomes. The findings are striking.

Private obstetricians had a spontaneous labour rate of 8.2%. Private midwives: 82.3%.

The two midwifery models were the only ones where more than half of women had spontaneous vaginal births. Standard hospital care had the highest rates of complications for babies, with 1 in 4 experiencing fetal distress and almost 1 in 5 needing special care or NICU. Private midwifery had the lowest.

Babies in standard hospital care were 10 times more likely to be given formula than those cared for by private midwives.

Women cared for by midwives received the most home visits, felt the most supported, and had fewer traumatic births. The study also found fewer traumatic births with private obstetricians suggesting that what reduces trauma is personalised care, continuity, and alignment between a woman's preferences and her model of care.

More medicalised does not necessarily mean safer.

The study concludes that denying women midwifery continuity of care is likely to reduce the quality of maternity care and increase negative outcomes for both women and babies.

This is Australian data but the questions it raises apply here. ARM has been campaigning for continuity of midwifery care since 1976. The evidence keeps saying the same thing.
